Registration with the Practice
Coggeshall Surgery is now accepting new registrations in the immediate Coggeshall area.
Patients are registered with the practice and not a specific doctor, however, you will be assigned a GP accountable for your care. You are requested to state a preferred doctor when registering and wherever possible we will try to make appointments with her. This helps the doctor to get to know you and provide continuity of care.
To register you will need to fill in a registration form called a GMS1 Form, please see the link below:

Please print off both forms and bring them to the surgery. If you are completing an adult form, please also bring proof of ID (passport, driving licence, birth certificate) and proof of address with your registration form.
If needed you may be asked to to make an appointment with our practice nurse for a new patient medical e.g. if you are on medication. She will take a note of your height, weight, blood pressure and test a sample of urine (specimen containers available at reception) etc.
