Registration with the Practice
Patients are now registered with the practice and not a specific doctor. You are requested to state a preferred doctor when registering and wherever possible we will try to make appointments with him/her. This helps the doctor to get to know you and provide continuity of care.
When you register you will also be asked to fill in a registration form and a medical questionnaire. This is because it can take some time for us to receive your medical records.
Before your records arrive and within three months of registration, please make an appointment with our practice nurse for a new patient medical. She will take a note of your height, weight, blood pressure and test a sample of urine (specimen containers available at reception) etc.
You may print off a registration form and new patient questionnaire by following the links below. Please fill them in and bring them in to the surgery. (Note: You need to use the File, Print menu to print these off)

You may print off our New patient Questionnaire or you may pick the form up from Reception. One is needed for each member of the family over the age of 5 years. If registering an under-5 with a complicated history, it would however be helpful to have a form completed.
Please refer to the ethic group codes when filling in the new patient questionnaire.
There is an additional form required for registration of a child or young person under 19 years. Please pick up or print off a copy, complete and return it to us with the other forms.
